We are using a new online service called Register with a GP Survey that makes it easy to register with a GP Surgery.
Just fill in this quick online form to start the process. You do not need proof of address or immigration status, ID or an NHS number.
The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.
Paper forms are still available if you need one. You can download the form here.
It would also be helpful (but not essential) if you can supply evidence of:

Identification - you are who you say you are, eg your passport, photo driving licence, birth certificate.

Address - this can be a bank statement, utility bill or other correspondence which places you in residence at your address.
Each GP practice has a geographical boundary, and people who have an address within the boundary area may apply to register with that practice. Our boundary includes, Wickersley, Ravenfield, Braithwell, Bramley, Sunnyside, Flanderwell, Micklebring, Dalton Magna parts of Hellaby & Maltby. Please use the interactive map at the bottom of this page to check if you live inside our practice boundary.
For economic migrants, asylum seekers, foreign visitors to the UK and ex-pats returning to the UK after living abroad, there are different rules. If you think that you may come under one of these categories, please contact our reception and they will give you a guide to help you through registration.

Accountable GP
The Government requires GP Practices to allocate a Named Accountable GP to all their patients from April 2015.
All of our patients have now duly been allocated a Named Accountable GP but can still continue to see any GP of choice within the Practice.
If you wish to know the name of your Accountable GP please ask when you are next in contact with the Practice.
