There are strict rules for registration at a GP practice. You will be asked to complete a registration form and a new patient questionnaire. You must also supply evidence of:
1. Identification - you are who you say you are, eg your passport, photo driving licence, birth certificate.
2. Address - this can be a bank statement, utility bill or other correspondence which places you in residence at your address. Each GP practice has a geographical boundary, and people who have an address within the boundary area may apply to register with that practice.
For economic migrants, asylum seekers, foreign visitors to the UK and ex-pats returning to the UK after living abroad, there are different rules.
